Job description
Job responsibilities
1. To work autonomously as a clinical specialist nurse/AHP to provide advanced assessment, clinical diagnosis and treatment of patients within the acute frailty pathway.
2. This post will work primarily within the emergency care floor and in-reach into acute wards, liaising appropriately with acute therapy and nursing teams in order to assess needs, providing same day services, as well as supporting the effective use of alternatives to acute care including SDEC, virtual wards and community rapid response services.
3. To support (and create where required) the delivery of frailty awareness activities across acute service.
4. To support and deputise for the Acute Frailty Consultant Practitioner with the operational leadership and overall development of the team, especially in regard to all aspects of clinical governance, ensuring that the service maintains high standards of care that are effective, sustainable and resilient.

Qualifications
Essential
1. Physiotherapy/Occupational Therapy Degree/Diploma.
2. HCPC Registration.
3. Evidence of CPD at a postgraduate level, reflective practice, IST.
4. Experience of working as part of a therapy team/multidisciplinary team.
5. Extensive general and acute experience.

1. Demonstrated ability to complete complex physical skills involving the moving and handling of patients and equipment.
2. Demonstrated ability to exert moderate physical effort for several short, and occasionally long, periods during the working day.
