The role ofHealthcare Assistantis fundamental to the success of Community Nursing Teams. The post holder will be able todemonstratethe competenciesrequiredto perform at this level and role model the Trust’s core valuesaswell asexhibitthe behavioursrequiredas part of those values.
• You will visita number ofpatientson a daily basisin their own homes.
• You will provide high quality nursing care to a range of patients with varying healthcare needs.
• Implement,monitor, andmaintainhigh standardsof care delivery at all times.
• Communicate effectively within your team, the patients and theircarers, and the wider MDT.
• Deliver high service standards, sharing best practice, and actively improving the way we work.
• Maintainsaccurateandtimelypatient records in line with organisational requirements.
• Undertake specific clinical tasks, when trained, which include:
• Completing a set of observations including, blood pressure, temperature, oxygen saturation, respiratoryrateand heart rate
• Completing wound care, using a wide range of dressings including bandaging, compression hosiery and topical cream application
• Pressure area care asappropriate, which includes ordering equipment.
• Venepuncture
• Catheter care – Administering catheter flushes and changing catheter bags.

1. Hold a full UK Driving Licence and have use of a car for work purposes.
