We are looking to recruit a Band 5 community staff nurse into the Romsey Integrated Care Team. You can make that difference and contribute to the provision of high-quality care, within a variety of community settings which include the patient's home, IV therapy and catheter clinics. Learn new skills including end of life care, wound care and holistic care planning.
The multi-disciplinary Integrated Community Teams work alongside primary care and a variety of community services to support and manage the care of housebound and high-risk patients. The scope of your role will focus on supporting admission avoidance, early supported discharge, promotion of independence and assisting in the effective management of patients with long term conditions.
The service runs from 8am-8pm, 7 days a week, 0830-1630,12-2000 and therefore this position requires flexible working, but come along and ask us - what hours would suit you?
All applicants require a driving license, professional registration and full access to a vehicle for travelling (mileage allowance payable).
Romsey and The New Forest offers a wide range of activities, as well as incorporating nearby stunning coastal areas, and your daily visits will be driving around in our beautiful countryside, local towns and villages.
Main duties of the job
Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, mental health, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.
The post holder will have responsibility for managing, co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
