To be responsible for the provision of paramedical services, working as part of the practices multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The paramedic practitioner will be required to work autonomously in the practice and in the community (home visits). To oversea the triage board on a day to day basis to provide patients access to a clinician for on the day advice. Oversea our housebound patients by visiting at their request but also completing any essential care checks with them. Liasie with both external sources to provide help and support to patients and also work with the staff within our network and practice. as part of an multidisciplinary team. Work alongside our clinicians to provide on the day care and support our clinicians with regular reviews, assessments and referrals. To attend meetings as part of a multi disciplinary team and palliative team to offer shared care. Provide a range of services to include assessments, diagnosis, treatment, telephone triage, and referring patients appropriately as well as supporting the management team in the reviewing of clinical policy and procedure.
