Job summary Mon - Fri 9-5 pm To lead on the monitoring and development of Home Enteral Feeding, in collaboration with CLCH and NCL colleagues for the safe and effective management of Home Enteral Feeding in the community. To support Malnutrition and Obesity caseloads as required. To provide education and training to dietitians, dietetic assistants and other healthcare professionals (HCPs) across the local community to facilitate continuous learning and development. To collaborate with primary care partners in best practice management of Home Enteral Feeding with reference to national and local guidance, recommendations and protocols. To engage and support primary care and community health the appropriate prescribing of enteral feeds in line with NCL pathways and formulary. To work in collaboration with any external partners who may be auditing GP practices / Care Homes and supporting with appropriate prescribing To make recommendations to GPs on the appropriate prescribing of enteral feeds and oral nutritional supplements Main duties of the job Lead on the Nutrition Support Care Pathways for home enteral feeding in collaboration with CLCH and NCL partners Utilise project management skills and work in partnership with service users, other agencies and professionals to plan, develop, implement and evaluate enteral feeding intervention programmes Take into consideration Trust Strategic Objectives, national policies such as NICE guidelines for Nutrition Support in Adults, NHS England, Care Quality Commission fundamental standards of care and NHS London Procurement Partnership's Medicines Optimisation and Pharmacy Procurement. Work with Acute trusts, Primary Care Networks, North Central London (NCL) enteral feeding consortium group, and medicines optimisation teams to further develop prescribing guidelines. engage gp's and provide best practice to improve prescribing practices the implementation of best practice care plans. Lead on the development and implementation of clinical standards, policies and procedures for home enteral feeding and nutrition support. Work in conjunction with the Clinical Lead Dietitian in developing appropriate prescribing initiatives for development of the teams strategic direction, objectives and business plans in line with the Trust Objectives.
