An opportunity has arisen for a Nurse, Social Worker or Occupational Therapist to join the Blackburn & Darwen Community Mental Health Team. You must be a motivated individual, committed to promoting recovery and well-being for our service users as part of our multi-disciplinary team. Excellent communication skills are essential for the role. Previous experience of Community Mental Health work is desirable.
This post provides the opportunity for a forward thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships.
You must be a registered mental health practitioner, with excellent leadership and clinical skills and have some post qualification experience in clinical work with service users experiencing a range of mental health problems.
You will also need to have a working knowledge of the care programme approach (CPA), Dialog+, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.
Main duties of the job
You will be a mental health (MH) practitioner working within a multidisciplinary team and carry out a diverse range of duties.
As a Band 6 MH practitioner within the team, you will be regularly supported and supervised by the clinical lead/team leader, and you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
You will be required to undertake the administration of medications and safe storage of same.
You will actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and Code of Conduct. You are required to maintain links with your professional body and provide evidence for the revalidation process.
You will provide support to the nurse leadership within the multidisciplinary team and ensure the effective assessment, formulation, planning and monitoring of care given to service users.
You will exercise professional accountability and responsibility using skills, knowledge and expertise in changing environments, across clinical boundaries and in unfamiliar situations as outlined within the NMC Code of Professional Conduct.
You will work flexibly by covering the core community mental health team hours of Monday – Friday 9 am – 5pm, to meet the needs of the service and the wider network.

Education Qualifications
* Registered Mental Health nurse, Occupational Therapist or Social Worker with ongoing Professional Registration.
* Evidence of commitment to continuing professional development.
Knowledge
* Understanding of health and safety policies and procedures.
* Knowledge and practice of Mental Health Act 1983.
* Can provide evidence of recent professional development.
Experience
* Willingness to participate in mentor and preceptorship process in respect of own practice.
Skills and Abilities
