Countywide - We have Occupational Therapist opportunities within Taunton, Sedgemoor & Yeovil.
We are looking for dynamic and inspiring Occupational Therapists to join the Discharge to Assess (D2A) service, part of the wider Intermediate Care service for Somerset. You will gain an in-depth understanding of community working and supporting people within their own homes. Successful candidates will be directly supported by a Senior Occupational Therapist and Advanced Practitioner/Clinical Specialist within the team. A vast number of people have been helped to regain their independence at home following a stay in the hospital thanks to Somerset's Discharge to Assess service.
Main duties of the job
You will be expected to carry out specific therapy assessments which will include goal setting and treatment planning, assessing risk, promoting and enabling independent functioning. You will be committed to a person-centred approach to your work, have the ability to prioritise and work flexibly, working alongside our provider teams and rehabilitation practitioners.

Person Specification
Qualifications
* Diploma/Degree in Occupational Therapy (OT)
* Registration with Health Care Professions Council
* Knowledge of community occupational therapy / Physiotherapy role
* The post holder must have excellent verbal and written communication skills to deal with the complex needs of these patients, carers and relatives, and may be exposed to situations and environments that may cause distress.
* To be able to use advanced clinical reasoning to predict achievable outcomes of treatment and to recommend the best course of intervention, developing comprehensive patient-centred treatment plans, in conjunction with other professions and agencies.
Experience
* Experience in assessment, treatment, and rehabilitation of a wide range of conditions
* Advanced clinical reasoning to enable sound judgement and intervention when treating patients
* Ability to work autonomously and set priorities
* Post registration experience as an Occupational Therapist including experience in a ward setting to include treatment of those with physical and mental health conditions
