An opportunity has arisen in the Medicine Division to join our team within Stroke Services on Ward C4 as an Activity Co-ordinator.
We are looking for someone who is dedicated and committed and has the ability to be part of a team as well as working autonomously to provide high quality dignified care to our patients in a safe environment.

Work as an activity co-ordinator with in-patients on the ward, under the guidance and supervision of qualified nursing and therapy staff.
Develop the provision of a varied and ongoing programme of individual and group based evidence based activities for in-patients.
Will maximise the range of evidence based activities that can be provided on the ward, in order to improve patient experience of hospital stays, to maintain or rehabilitate in-patients’ physical and cognitive skills, and to contribute towards discharge planning processes.
Will work alongside others engaged in the delivery of meaningful activity on the ward, in order to plan, deliver, and evaluate patient activity programmes.

To plan, prepare and carry out the appropriate level of meaningful activity with patients, and to progress activity programmes under appropriate guidance from qualified therapy staff.
Communicate effectively with patients, carers/relatives and all multi-professional colleagues regarding all aspects of patient care.
Utilising tact and persuasion engage patients in activity programmes, making basic clinical decisions e.g. is the patient fit to undertake the planned activity.
