We are looking for an Adult Nurse Manager to join a well-establishedprovider of complex care packages within the community, specialising in adult complex care. This community role will be based in the West London catchmentarea where the company continues tosecurefurther packages due to their excellent care standards.The successful Adult Nurse Manager will be responsible for providing face to face support to adult clients and their families, whilst creating personalised care plans for each person. You will work closely with a core team of carers, training and supporting them with their clinical skills and activities.Criteria for aAdult Nurse Manager:RGN with an active NMC PINSupportive management style that can bring out the best in othersUp to date experience of tracheostomy care and ventilators within the last 2 yearsAdvanced communication skills verbally and written to liaise effectively with patients, families and the MDTSuitable forNurses with evidence of effective leadership and management of HCAsFull UK Driving LicenceessentialKey responsibilities for a Adult Nurse Manager: Design and deliver a tailored care plan specific to an individual clients needsEffectively manage the MDT through strong communication and teamworkTo provide ongoing monitoring, support and assessment of clients physical and physiological needs within your caseloadTo deliver nursing interventions including the management of medicine deliveryBuild and maintain sophisticated relationships with your clients familyWorking closely with community teams, CHC nurses and CCG commissioners to establish and maintain bespoke packages of careBenefit package for a Adult Nurse Manager:£47,000 per annum DOEMonday to Friday (9-5) hours33 days holidayPrivate pension schemeUse of pool company car or travel card for community travelBusiness travel expenses of £0.45 per mileExcellent Learning and Development programmeCompany Mobile Phone and LaptopThis role will suit any experienced RGN looking to further your career and join a stable, market-established provider.
