We have some exciting opportunities for enthusiastic and experienced professionals with experience of working in Paediatric services who are looking to move into a more senior role to develop leadership skills whilst maintaining an operational role. This is a new role within the CYPCC service recognizing the challenges and complexities faced by children and young people with complex needs.
Main duties of the job
You will be responsible for managing a caseload of eligible children and young people, assessing and reviewing packages of care and support. You will have strong relationship-building skills and be a key point of contact for children, young people, and their families.
You will work collaboratively with the child or young person and their families and carers to produce a personal health and support plan, working with system partners to ensure the delivery of holistic packages of care that meet identified care and support needs.
You will work alongside Adult Services colleagues to ensure a positive, young person-focused transition to adulthood.
You will take part in the triage of new referrals and undertake assessments of eligibility for Children and Young Peoples Continuing Care with a multi-disciplinary team, presenting recommendations to the panel.
As a new part of this role, you will also be responsible for designated key strategic areas in relation to the delivery of Children and Young Peoples Continuing Care in Dorset. This will involve working with stakeholders to improve system knowledge of CYPCC and individual experiences of the CYPCC service and strengthening current processes.

Job Description
Job responsibilities
The post holder will provide case coordination of healthcare packages at home and other community settings for children and young people who are eligible for NHS children's continuing care support. The post holder will implement and assist in the coordination of multi-agency packages of care between health, social care, and education.
The post holder will work in partnership with children/young people (CYP) and their parents/carers to formulate effective care packages which meet the identified needs, facilitating early discharge of children and young people from hospital and reducing or preventing hospital re-admission.
The post holder will provide expert leadership within the team, contributing to the setting and delivery of high standards of care to children and young people who are eligible for NHS continuing care.
The post holder will have a high degree of knowledge of legal frameworks surrounding the care and support of a child/young person including Mental Capacity Act, Children Act, and NHS Continuing Care framework.
The post holder will lead the development of the service in agreed key areas (e.g., triage and assessment, Dorset Place, Bournemouth Christchurch and Poole Place or decision making, appeals, and transitions) and be responsible for the education of CYP, parents/carers, and staff and key partners in order to achieve the best health and well-being outcomes for every child who has an NHS continuing care package.
The post holder will build professional relationships with key partners including children's social care, NHS Services, and voluntary sector organizations to ensure the needs of the child/young person are met effectively.
The post holder will play a key role in supporting the Operations Manager in managing and developing the NHS Dorset Continuing Care team, working in conjunction with external stakeholders, to implement a local vision of services, ensuring the delivery of key quality outcomes and strategic objectives for the service and ICB.
The post holder should hold a full UK driving license and have access to a vehicle to be able to fulfill the requirements of this post.
Dependent on key service responsibilities, the post holder may also be responsible for the line management and supervision of up to two associated staff within the service.
A flexible, supportive, and proactive approach is required as the role will need to adapt to ensure the needs of the service are met.
Person Specification
Experience Essential
* Extensive practice experience working with children and young people with additional health, social, or educational needs.
* Extensive knowledge of specialist areas acquired through experience and training plus further specialist knowledge of the Personal Health Commissioning process.
* Understanding of Personal Health Commissioning issues such as consent issues, safeguarding, etc.
* Line management experience and/or a relevant qualification.
* Experience of working in a health environment.
Qualifications Essential
* Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in the specialist area, ideally Personal Health Commissioning.
* Children's Nursing qualification or Registered Learning Disability Nurse, Registered Mental Health Nurse, Occupational Therapist, Speech and Language Therapist, or Physiotherapist with recent paediatric experience registration with the relevant professional body. Social Worker with relevant Children's experience.
* Knowledge of NHS National Framework for Children's and Young Peoples Continuing Care associated procedures and practices, underpinned by theoretical knowledge of other relevant legislation.
* Evidence of Continued Professional Development.
