Cornwall Partnership NHS Foundation Trust has an exciting opportunity available for an enthusiastic and highly motivated individual to join our Community Mental Health Team based in Liskeard.
North Cornwall Community Mental Health Team works across East Cornwall. The team strives to deliver high quality and effective services to people experiencing severe and enduring mental health problems, working as a multidisciplinary team. We offer assessment and treatment of mental health problems, with a focus on effectiveness and recovery, working to promote the independence of our service users. The team are looking for someone who can demonstrate the Trust's vision and values in practice, and who is motivated to be involved in innovation and service development and passionate about mental health.
We are looking for someone with a core profession in mental health nursing and who has experience in working independently in a mental health setting.
As a CMHT team member you must have both a positive outlook and excellent communication skills. You will need to have knowledge of general mental health problems, the impact these may have on service users and their treatment.
Main duties of the job
1. To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
2. To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising dialog+ and other outcome measures.
3. To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
4. You will need to have knowledge of Dialog+, legislation around mental health and The Mental Health Act.
5. You will need excellent communication skills and place value on team and inter-agency working.
6. We would like to work with an individual who wants to utilise their skills to effect positive changes for our service users.
7. We need a person who can demonstrate passion and commitment to mental health issues, and who can provide excellent client centred care along with working flexibly as part of a team.
8. You will receive regular support and supervision from the Management and Leadership team.

Person Specification
Qualifications & Experience
* Current professional registration - HCPC RMNC
* Evidence of post basic training
* Extensive experience of working with mental health patients
* Demonstrate clinical leadership skills
* Professional qualification - OT, Social worker, RMN
Knowledge and Skills
* Sound working knowledge of the Mental Health Act of 1983 and other mental health legislation
* Ability to supervise staff, demonstrating the highest clinical standards
