We are seeking a skilled and compassionate Practice Nurse to join our dynamic team, experience in primary care or community nursing would be preferable. We are ideally looking for someone with experience in the management of Diabetes, cervical screening procedures, and childhood immunisations but training can be provided. Excellent communication and interpersonal skills are essential as is the ability to work effectively both independently and as part of a team. Main duties of the job - Provide nursing care and support to patients with Diabetes and other Chronic Diseases including education on self-management and lifestyle modifications. - Conduct cervical screening and follow-up procedures to ensure patient compliance and health monitoring. - Administer childhood immunisations in line with national guidelines and support parents with immunisation queries. - Collaborate with the healthcare team to develop and implement patient care plans. - Maintain accurate patient records and ensure confidentiality at all times. - Participate in health promotion and education initiatives within the practice. - Any other Practice Nurse duties relevant to the role. Assist in the provision of nursing services for the practice to include the delegation of duties to other practice nursing staff as appropriate. Assist the nursing needs of patients and provide appropriate care-treatment in conjunction with team members, the GP or independently, as appropriate and in accordance with practice policy and protocols.
