Job summary
An exciting opportunity awaits the right candidate to work within an extremely supportive clinical team based in Dorchester. Our team of nurses and paramedics are led by an experienced Advanced Nurse Practitioner in providing high quality and personalised care to patients in their own homes and care homes across Mid Dorset.
We are looking for maternity cover for an initial period of six months, which may be extended to one year.
The role is advertised as ideally full time, but we would consider part time for the perfect candidate.

Main duties of the job
You will have a clinical role working autonomously with supervision and support to assess, manage and monitor the health needs of patients. The post holder will work collaboratively with the Frailty Team, Care Home Teams, GPs, Community Nursing Team, Rehabilitation Teams, Community and Acute hospitals.

Job responsibilities
1. Be competent at identifying at risk patients and adding them to the caseload.
2. Proactively monitor and manage an identified patient caseload of patients within care/residential homes.
3. Empower patients to self-manage their care at appropriate stages and support them to cope with changes.
4. Work with patients and carers to develop and review personalised treatment, care and management plans.
5. Provide advice and information to assist individuals to make lifestyle changes and to manage and monitor their frailty.
6. Support and manage patients care at the end of their life.
7. Ensure that patients are appropriately referred to other practitioners when it is in the best interest of the individual needing action, care or treatment.
8. Ensure that help is requested from a suitably qualified and experienced healthcare professional to carry out any action or procedure that is beyond the limits of your competence.
9. Work in partnership with secondary care and Community Hospitals to facilitate the planning of care and services on discharge for case-managed patients.
10. Work in partnership with clinicians with specialist skills and knowledge to meet the needs of people with complex long-term conditions (including frailty).
11. Actively work towards reducing hospital bed days and innovative new ways of working to provide more care in the community.
12. Take all reasonable personal precautions necessary to avoid any potential health risks to colleagues, people receiving care and the public.
13. Remain objective and have clear professional boundaries at all times with people in your care, their families and carers.
14. Safeguard and promote the welfare of vulnerable adults, children and young people in all areas of work.
Person Specification
Qualifications Essential
* Registered Nurse
* Experience of Care Planning for older people
* Experience of holistic assessment of older people
* Clinical skills in clinical observations and phlebotomy
Information Technology Essential
* Working with IT systems, including Word, Excel and Access.
* The ability to handle sensitive information confidentially.
Desirable
* Experience in using clinical systems e.g SystmOne.
Experience Essential
* Experience of working under own direction.
* Experience in the ability to self-motivate, organise and prioritise workload.
Desirable
* Previous experience working in a primary/community care setting in a similar role.
* Experience of supporting service improvement.
Personal Attributes Essential
* Excellent written and verbal communication skills.
* Using own initiative and work safely in lone working environments.
* Demonstrable skills in written and spoken English, adequate to enable the post holder to carry out the role effectively.
* Subject to the provisions of the Equality Act, able to travel using own vehicle on Network business.
