To work as part of a Specialist Children and Young People’s Mental Health Team to provide intensive Tier 4 support and interventions for young people and their families.
The post will support Senior Mental Health Practitioners in delivering safe and effective care to young people and their families, as agreed in their care plan.
The post will involve working under the supervision of a senior member of the team but will include independent work in the young person’s home and other community settings.
The hours of operation for the Children and Young People’s Mental Health Community Intensive Support Team will be variable and will require out of hours working that will be 7 days per week.
Responsibilities include:
1. Providing support for young people and their carers/families to enable them to achieve collaboratively agreed goals defined in their care plan.
2. Supporting the development of relapse prevention plans that focus on the young person and families' strengths and proactive approaches to maintaining positive wellbeing.
3. Promoting young people’s social engagement, health and wellbeing, and age-appropriate independence.
4. Actively supporting young people to access appropriate local community resources.
5. Promoting service users achieving their goals in terms of working towards recovery.
6. Creating and maintaining effective working relationships with external organisations and agencies.
7. Keeping accurate and current clinical records.
8. Supporting the service user in care planning and promoting independence.
9. Forming positive therapeutic relationships that support service users’ own decision-making and build on their strengths.
10. Supporting service users in creating a crisis and contingency plan wherever appropriate.
