We are looking to recruit to 3 x Social Care Practitioners role (fixed term/ full time) based at various D2A locations in Bristol and 1 x Social Care Practitioner role (permanent/ part time) based within the Discharge to Assess North Team.
We are looking for individuals who recognises the value of multidisciplinary working and are keen to progress the model of service delivery.
The D2A Service, work together to promote a whole service approach, with our internal & external colleagues.
The work is fast paced, this includes Safeguarding Adults enquiries, Care Act assessments, a good knowledge of local services and voluntary agencies, including tier 1 & 2.
Your role would be to work under the statutory requirements of the Care Act 2014 to undertake support conversations, strength based Care Act assessments, Carers Assessments and reviews with citizens in need of support and help them to access appropriate support in line with the department’s policies and procedures under the supervision of a Manager, Senior Practitioner or Social Worker.
You will be required to undertake support planning with service users who has eligible care and support needs to access the support they need and to meet agreed outcomes that promote choice and control and help them remain independent.
As part of your role, you will be undertaking Mental Capacity Assessments and Best Interests within the statutory requirements of the Mental Capacity Act 2005 with the support of qualified Social Worker where required.
The teams have a make-up of 1 team manager, 2 senior practitioners, 6 FTE social workers & 6 FTE social care practitioners. You will have regular formal reflective supervision by either manager or senior practitioner.
You will ensure citizens have the right professional expertise for their presenting needs at the earliest opportunity: preventing or delaying the need for care and support through maximising independence, autonomy, community engagement, managing or enabling risk, and health promotion.
You will have a full induction and opportunities for training and continuous professional development activities. The teams are primarily based in Bristol, we also operate an agile working policy which allows for home working.
