We are looking for an enthusiastic and dynamic Band 3 Reablement Assistant to join us on the new Richard bright Community ward based at St Helier Hospital in Carshalton.

Main duties of the job
The successful applicant will be working alongside physiotherapists, occupational therapists, nurses, pharmacist and a GP to deliver high quality patient care and rehabilitation enabling patients to achieve their therapy goals and promote independence prior to discharge home.
Key responsibilities will include carrying out therapy plans as per OT/PT recommendations and working as part of the MDT to ensure patients safe and timely discharge home.
Central to this role are:
1. Strong communication skills
2. Experience of working autonomously and as part of a team
3. Background experience within health or social care
4. Interest in rehabilitation

SHC vision is to deliver seamless excellent coordinated care that is person centred and of high quality care for people at risk of an unrequired hospital admission, and for those that required admission ensure they are supported promptly to get them home again as quickly as possible after hospital admission.

Essential criteria
5. NVQ Level 3 health related study or equivalent experience
6. Written, verbal and numeric skills to GCSE or similar level
Desirable criteria
7. Cavendish Care Certificate

8. Experience of a caring role
9. Experience of working as a member of a team within health or social care
10. Experience in patient documentation
Desirable criteria
11. Worked as a therapy or healthcare assistant
Skills/Abilities/ Knowledge

12. Demonstrate ability and willingness to learn new skills
13. Basic IT Skills. e.g WORD, Outlook, Emails
14. Demonstrate the understanding of the role of rehabilitation assistant

15. Demonstrate organisational and time management skills
