Bolton Community Mental Health Team (CMHT) is an integrated health and social care service that strives to deliver a needs-based service for people with severe and enduring mental health conditions. Functional CMHT’s are part of Bolton Division of Greater Manchester Mental Health NHS Foundation Trust and provide a comprehensive multi-disciplinary service to people aged 18 years and above, including an assertive outreach approach.
This is an exciting opportunity for support, time and recovery (STR) worker to join theFunctional Community Mental Health Team in Bolton to provide a comprehensive multi-disciplinary service.
The purpose of the post is to work with people living in the community who are experiencing a deterioration in their mental wellbeing; this will include working within the service user’s own home. This will be achieved by working directly with service users to provideSupport and giveTime to engage them in theirRecovery and promote health and well-being in line with their care plan, under the direction of the Care Coordinator/Occupational Therapist/Team Leader.
