Job summary We are looking to recruit an experienced and highly motivated advanced clinical practitioner, either a nurse or allied health professional (AHP), to join the Frailty Virtual Ward team. The role will primarily involve working closely with the clinical lead, liaising with general practitioners and community teams to assess patients who meet the criteria for the frailty virtual ward. The post holder will contribute to the development of individualized care plans and provide expert management of medical conditions, ensuring patients receive high-quality, personalized care. Additionally, the post holder will apply their expertise to safely manage patients in the comfort of their own homes, helping to improve outcomes and enhance the quality of life for those with frailty. Main duties of the job Contribute to the development, implementation, and review of personalized care plans for frail patients, ensuring they are tailored to individual needs and preferences. Work closely with the clinical lead to adjust care plans as required, ensuring optimal management of both acute and long-term medical conditions. Promote a holistic approach to care, considering both medical and psychosocial aspects of the patient's health. Provide advanced clinical decision-making, diagnosing, and prescribing within the scope of practice to manage medical conditions and frailty symptoms effectively. Monitor and manage frailty-related conditions, such as falls, mobility issues, cognitive decline, and chronic disease management, ensuring patients' needs are met in both acute and community settings. Liaise regularly with general practitioners, community teams, and other healthcare providers to ensure continuity of care for frail patients. Provide professional guidance and support to community teams and junior healthcare staff in managing frailty within the virtual ward framework. Utilize digital tools and remote monitoring systems to track patients' conditions, providing virtual consultations and interventions when appropriate.
