An exciting opportunity has arisen to support the Hospital Discharge Team to improve the discharge process and support flow through the organisation. We are looking for a motivated and passionate person to join our team. The post holder will be based within the Hospital Flow structure, working within the acute setting across BHRUT Hospitals. They will be responsible for supporting discharges within inpatient wards.
They will be expected to work closely alongside the Multidisciplinary Team (MDT), patients and families, our Integrated Discharge Hub, and community partners to ensure smooth and safe discharges. They will join the existing team of Discharge Coordinators to support the planning of timely and appropriate discharges, working to support the Trust goal to increase the number of discharges taking place before 12 noon. The role will involve focusing on patients that no longer meet the criteria to reside in an acute bed. They will work as part of the ward team to focus on early identification of discharge planning and potential complex needs. They will be supported by the wards and the Hospital Discharge Team. This role may include weekend working.
The Discharge Coordinator role provides support to the clinical areas ensuring patients requiring discharge support are progressed through the appropriate pathway, working with inpatient wards, patients, families, and community partners.
The post holder will work within the Hospital Flow Team at Queen’s or King George Hospital, and Clinical Care Groups with a particular focus on ensuring patients progress through the appropriate discharge pathway supporting the Trust to provide, safe, well-governed, productive, and cost-effective services which support the Trust in delivering its strategic objectives.

These CDCs are open 12 hours a day, 7 days a week and are providing an extra 88,000 scans a year.
