The post holder will commission highly personalised care and support packages to enable people to live meaningful lives in the community in settings which are sustainable, high quality and safe, living as part of the community to allow people to thrive and live their best life.
The Clinical Commissioner will utilise and adhere to nationally recognised tools including, the 12-point discharge plan, Care (Education) and Treatment Reviews (C(E)TRS), 6-8 week checks and locally agreed processes and protocols including provider brokerage and frameworks, housing protocols and funding panel processes, ensuring packages are personalised, high quality and financially effective.
