Senior Mental Health Practitioner – Band 6, Mental Health Liaison Team:
An exciting opportunity has arisen for a Senior Mental Health Practitioner within the Mental Health Liaison Team based at the Harplands Hospital. This vacancy would suit someone who is looking for an exciting opportunity and wants to work as part of a successful Team.
Our Mental Health Liaison Team is part of the Crisis Care Centre at Harplands Hospital, Stoke-on-Trent, designed to the highest level of specification. This modern purpose-designed unit will be the central point for all those in need of a Crisis Assessment/Intervention.
The post holder is responsible for the delivery of care and for the supervision of others who are carrying out all aspects of the caring process. The post-holder will assist in setting and maintaining standards of care, participate in maintaining effective management of the team and unit, and maintain effective integration with the multi-disciplinary team, assisting with the facilitation of a collaborative approach to patient-focused care.
The post-holder will act as a reliable role model for junior staff.

Key Responsibilities:
1. To act at all times in accordance with the Nursing & Midwifery Council (NMC) code of professional conduct or relevant professional body.
2. To perform all aspects of the role in accordance with Trust policies and other national bodies.
3. To participate in the assessment of care needs and the development, implementation, and evaluation of individual care plans.
4. To provide specialist advice to other members of staff and act as a role model.
5. In conjunction with the Team Manager, to take responsibility for setting, maintaining, and evaluating standards of care throughout the 24-hour period.
6. To liaise with all members of the multi-disciplinary team and outside agencies in the implementation of patient care.
7. To ensure that care activities are research-based and in keeping with changes in the field of activity.
8. To contribute to the development of ideas and innovative practice.
9. To initiate and participate in departmental audit as requested by the Team Manager.
