Invicta Health Community Interest Company is delighted to be recruiting for a Care Co-ordinator, across our three GP practices in Romney Marsh, New Romney and Lydd.
Main duties of the job
As a Care Co-Ordinator, you will provide an important role in supporting the smooth running of clinics by performing routine administration tasks, combined in some areas with basic clinical tasks on behalf of the GP, allowing them to spend their time focusing on appointments and improving patient experience in the surgery.
As a Care Homes Care Co-Ordinator, you will be responsible for helping to improve the continuity of care by acting as a point of contact for residents, families and professionals who visit, or work in the care home, such as multi-disciplinary team (MDT) members and in-reach specialists. You will work to provide extra time, capacity and expertise to support individuals in preparing for, or in following-up clinical conversations they have with primary care professionals. You will work closely with primary care professionals within the PCN to identify and manage a caseload of identified care homes; making sure that appropriate support is made available to the staff and their residents; and ensuring that their changing needs are addressed. You will focus delivery of the comprehensive model to reflect local priorities, health inequalities or population health.

Invicta Health was formed in 2008 with the aim of supporting primary care services in the local area. We currently operate a number of General Practices within the Romney Marsh, Dover, Birchington, Broadstairs and Ramsgate areas. Invicta Health manages other services including the Primary Care Mental Health Specialist service which is delivered across east Kent and Swale. As part of a multi-organisation alliance, Invicta Health also provides General Practitioners and clinical support for the Urgent Treatment Centres based in Kent & Canterbury Hospital and Buckland Hospital (Dover).
Invicta Health is a Community Interest Company limited by shares. This means that the company has not been established or conducted to make private gain. The majority of any profit made needs to be available for investment back into the company or for the benefit of the community.
We are a shareholding company and can only distribute a small proportion of this to the shareholders and this will only happen if the organisation is confident that doing so will not impact on the care provided. We currently support a number of local community services and invest into our services and sites on a regular basis.
We have developed a number of partnerships ranging from our local acute trust, community and mental health trust, local federations, out of hours providers, practices and third sector organisations.
Job responsibilities
Main Responsibilities
1. Sorting all clinical post and prioritising for the GP in terms of actions. Signposting some post to others such as clinical pharmacist where necessary.
2. Extracting all information from clinical letters that need coding and summarising and action accordingly.
3. Arranging appointments, referrals, tests and follow-up appointments of patients.
4. Identify triggers and urgent situations and refer to the correct healthcare professional as appropriate.
5. Completing basic forms and core elements of some forms for the GP to approve and sign such as insurance forms, mortgage, benefits agency forms etc.
6. Explaining treatment procedures to patients, including arranging follow-up appointments.
7. Helping the GP liaise with outside agencies, for example getting an on-call doctor on the phone to ask advice or arrange admission while the GP can continue with their consultations.
8. Work within a multi-disciplinary team, providing support in arranging and making adjustments to work tasks and schedules as necessary.
9. To support the practice with QOF and associated claims.
10. Monitor clinical and organisational targets, reviewing the practice's performance against QOF compliance on a monthly, quarterly, and annual basis, liaising with the Practice Manager to address any areas of potential or actual underperformance.
11. Identify and claim all available income for the practice through the various claims systems accurately and in adherence to deadlines.
12. Monitor APEX, CQRS and PCQS claims monthly, working to identify trends and investigate any anomalies. Reporting to the management team when requested.
13. Produce accurate monthly reports on practice QOF and Enhanced Services (ES) claims and present these at management meetings when requested.
14. Conduct searches and reports as required using the EMIS system.
15. Proactively identify and work with a group of care homes to support personalised care requirements for their residents, using the available decision support aids.
16. Help people to manage their needs, answering their queries and supporting them to make referrals into the Care Home MDT or escalation of need to the Care Home Clinical Team.
17. Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.
18. Lead the coordination and delivery of the Care Home MDTs within the PCN.

Qualifications
* Educated to A Level or equivalent or relevant work experience
* Higher education certificate
Experience
* Knowledge and understanding of different claim categories and processes within primary care
* Previous QOF experience
* Understanding the importance of maintaining patient and data confidentiality.
* Experience of working with Microsoft packages including Word, Excel and PowerPoint.
* Knowledge of read codes/SNOMED codes.
* Previous experience of processing complex requests
* Knowledge and experience in the use of the E-referral system (ERS)
* Experience of working with EMIS and DOCMAN
* Knowledge and understanding of the National Health Service
