Salary: £32,600 to £37,340 per annum, full time equivalent, dependent upon experience and relevant qualifications (Pro-rata for part time)
Hours: 22.5 – 30 hours per week, to normally be worked Monday to Friday, with flexibility to cover a 7 day service.

The role
You will be the point of contact for all Inpatient referrals and responsible for co-ordinating patient flow, from admission to discharge within the IPU. You will work within a multidisciplinary team fostering positive relationships with our Therapies and Hospice at Home teams. You will be a main point of support for patients, their families, carers and colleagues.
You will be confident in taking the lead at multidisciplinary team meetings, engaging in discharge discussions and holding the medical team accountable for their role in projecting discharge dates. Outside of the organisation, you will foster close relationships with key referrers and promote the Hospice to help widen our reach to more patients.

Skills and experience
We’re looking for a motivated and enthusiast individual who has:
• A registered nurse qualification
• Current NMC registration
• A minimum of two years post registration experience
• Knowledge of social care and continuing healthcare provision and funding
• Outstanding communication skills
Due to the nature of this caring role, an enhanced DBS clearance will be required.
