Attend daily board rounds- to be aware of all the patients on your allocated ward area.
To liaise with the Band 6 supporting your team, The role of the discharge Co-ordinator is to provide support to wards, attend board rounds as discharge starts on admission. Ensure that all patients are discharged safely with the care they require. To be able to escalate any delays in care or discharge planningTo liaise with other team members for patients from care homes and patients that are approaching end of life
