Collate information at the point of referral from primary care and in the pre-screening questionnaire. Use the above information and digital intelligence to support the initial triage of those likely to require optimisation. Contact patients to inform them of outcome of pre-screening triage, and where appropriate and under the direction of a qualified health professional, use approved materials to provide them with universal advice about pre-operative preparation, including diet, exercise/activity and healthy living. Under the guidance of a registered health professional, arrange appropriate follow up appointments for higher risk patients. Use this knowledge to refer patients to their most appropriate local services. Examples will include activity classes, weight loss and psychological support. Refer patients to health promotion services, for example smoking cessation, alcohol reduction and diabetes control. Where relevant, encouraging patients to find out if their GP has a social prescribing link worker and/or health coach, and how they can refer themselves. Quality assure surgical prioritisation coding in line with Royal College guidelines and where necessary, escalate and stream cohorts of patients to the relevant clinicians and services.
