Health and Wellbeing Practitioner
Job overview
An exciting opportunity has arisen for a Band 4 Mental health and wellbeing Practitioner to assist in the overall delivery of care with a ‘can do’ attitude, be resilient, and possess excellent organisational skills.
To work with Prison Service partners in providing appropriate support and risk assessments for patients who are subject to prison service risk management procedures such as ACCT and segregation procedures.
This role will involve delivering high quality care to all patients on a caseload through a process of coordination, planning, and implementation of specific individual activities.
By being involved in multi-disciplinary and inter-agency liaison, you will support Effective Care Coordination and the formulation of risk management strategies for those identified with complex needs. This also includes carers or significant others as necessary or required.
Main duties of the job
The Main Duties for this role are to assess the health and well-being needs of service users on a caseload, as directed by the relevant professional. Plan and deliver care and work with individuals’ strengths and interests and support clients to identify realistic goals in these areas.
Safeguard service user's and carers health and well-being ensuring that all concerns are reported.
Support the assessment and management of service user care needs through initial information collection, monitoring of service users progress, feedback discussion reports, and in care reviews with the clinical team.
To ensure the clinical area is safe, clean and follows infection prevention precautions. Completion of Venepuncture and ECG, making sure to communicate with the wider MDT any results received, in conjunction with client and wider MDT.
