We're a unique service with a skill mix team which includes: Health Visitors, Community Staff Nurses and Community Nursery Nurses.
Main duties of the job
* A qualified (or currently in training) as a Specialist Community Public Health Nurse
* Well versed in co-ordinating health promotion initiatives
* Experienced in assessing, planning and implementing programmes of care
* Used to working collaboratively as part of a multidisciplinary team
* Up to date on recent developments in primary care and health legislation

For registered Band 6 Health Visitors, this post is offering a premium of £2,500 payable upon commencement of employment (T&C's apply)
At CLCH we've been continuing to re-shape the way Health Visitors work. Throughout the pandemic we have kept our Health Visitors working safely with local families by introducing new and innovative practices to keep them safe, including agile working and provision of laptops and additional technological solutions.

Person Specification
Education/Qualification
* Registered Nurse/Registered Midwife
* Registered Health Visitor (SCPHN)
* Current NMC registration
* Evidence of continuing post-registration education and training
* Practice assessor/supervisor training or working towards
Experience
* Be able to co-ordinate health promotion initiatives in line with local and national health improvement targets
* Experience of working collaboratively as part of a multidisciplinary team and inter-agency working
* Experience of organising and managing own workload and that of others
* Experience of assessing, planning and implementing programmes of care and writing care plans
* Experience in providing practice-based learning support to others
* Experience of evidence-based care
* Experience of supervising staff / students
Key Attributes
* Able to travel round the locality [car/bike/public transport/foot]
* Able to meet the required IT Skills for the post
