Job summary We are looking to recruit new a Community Physiotherapist to join our friendly Therapy team in Amber Valley. We have a 30 hour per week vacancy, and an opportunity to work within a variety of our different workstreams to include Rapid Response, Higher Priority and Routine core. The hours of the service vary depending on the pathway but you will be required to work between the hours of 8.00 and 18.30. The post holder would be able to work condensed hours- working 3 longer days or 4 shorter days, but may be required to cover the later shift. You will be required to see patients in the Community which will give you the opportunity to adopt agile working between your own home and our various bases across Amber Valley. This offers great flexibility and independence for managing your own caseload. The Community Therapy team comprises of Physiotherapists and Occupational Therapists with band 4 and band 3 support workers. We are part of a large Integrated Community Team which include Community Nursing, Care Co-ordinators and the Community Response Team. You will be required to work weekends as part of a shared rota. This will give you the opportunity to see urgent patients as part of a wider team. You will receive enhanced weekend pay and days off in the week as a result. (Approx. 1 in 6 weekends) You will be joining an established team with a wealth of experience and you will have access to a Clinical Lead Physiotherapist who will be able to support you with your development. Main duties of the job You will be required to conduction comprehensive, holistic assessments in the Community setting. We like to view Community as a speciality in it's own right and you will be exposed to a range of conditions and scenarios which include Falls, MSK, Neurology, Respiratory, manual handling and equipment provision. This gives you the opportunity to develop your knowledge and skills across a wide area with an underlying focus on Frailty. You will be required to establish treatment plans and goals following your assessment and mange your own caseload. You will need to work with your Core Community colleagues to manage patient flow and maintain capacity within your diary. You will be required to attend and take part in meetings which will include Community Delivery Team (CDT) meetings with our GP practices, which will enable you to develop good working relationships with the relevant Care Co-ordinator for the practice that you are linked to. You will be required to do an array of mandatory training related to your role and we will identify any gaps in your knowledge and skills and support you to close these to gain your competencies as a Community Physiotherapist.
