This is a post for a pharmacist to join the Wandsworth Care Home Pharmacist Service working 15hrs per week on a permanent contract.
Central London Community Healthcare NHS Trust is looking to recruit a dynamic and innovative pharmacist to join the Medicines Management Team at Central London Community Healthcare Trust.
Working as part of the Medicines Optimisation Service for care home residents, ensuring an integrated approach to the management of medicines.
This will involve conducting clinical medication reviews in patients living in care homes who have multiple comorbidities and complexities. A knowledge of polypharmacy, long-term conditions, frailty, and medicines optimisation is essential. You will be acting as a source of pharmaceutical expertise to various health and social care professionals e.g., GPs, care home staff, hospital clinicians, district nurses, carers, families, and community pharmacists. Continuous evaluation of the service will also be key to demonstrate value and areas for further development.
Trust wide Medicines Management Service Provision including policy development, implementation of patient safety initiatives e.g., drug alerts, training for health professionals in the community, and support to the bedded services if required.
The post will predominantly be working in Wandsworth with a base at Parsons Green and will involve lone-working and travelling between sites. The postholder may also be required to travel to other sites and services within the organisation, so will require flexibility as the organisation continues to grow.
You would also be part of the on-call advisory service (by phone only) providing advice to Bedded services.

The postholder must have significant post-registration experience in providing clinical pharmacy services and a postgraduate qualification to at least diploma level or equivalent. A sound knowledge of prescribing and medicines management issues faced in both primary and secondary care is essential, with excellent decision-making and problem-solving skills. Additionally, compassion, flexibility, excellent communication and time management skills are essential, and the ability to build relationships with various health and social care professionals e.g., GPs, care home staff, hospital clinicians, specialist nurses, carers, family, and community pharmacists.
