We here at BHFT are looking for anOccupational Therapistto workwithin the Care Home SupportTeam.This unique role will allow you to work on a Monday to Friday basisbetween normal office hours, using yourclinicalexpertiseto train and developcarehome staff across East Berkshire.Unlike your normalclinicalrole, you will be working from home and taking occasional visits tocarehomes across East Berkshire to deliver in person training.
You willwork with the Integrated Care Board, CareHomesandLocal Authorities to ensure that our care home staff are delivering an enhanced and top tier service to its residents.

• To work in partnership with care homes to develop pro-active training/educating advisory service that aims to improve the health, well-being, and independence of all care home residents.
• Identifythe specific skills and training requirements for each care home and deliver and record a tailored training programme
• Attend and contribute to clinical/care homes MDT and cluster meetings whereappropriate.
• Share expert knowledge and skills with the care homes to enhance their skills.
• To independently manage a small caseload of complex cases relating to mobility, pressure, and postural care and often in combination with communication, challenging, cognitive and/or perceptual difficulties.
• Formulate individualised action plans based on the assessment findings and clinical reasoning.
• Work as a senior practitioner and be professionally and legally responsible for all aspects of your own work.
• Complete support visits to care homes and offer advice and support
• Continue working alongside local authorities to support those homes rated red or amber and those with residents who have frequent hospital admissions.
• Signpost and support with making direct referrals on to other community services or professions asrequired
