Job summary 1 x 37.5 hours per week We are looking to appoint a motivated & forward-thinking Senior Therapy Assistant to join the Neighbourhood Rehabilitation Service (NRS) which encompasses the Intermediate Care team and Long-Term Condition Pathway, working across Barnsley Community. The team provides rehabilitation to patients in their own home & residential care. The service provides specialist community therapy assessment & treatment to improve health & independence. In addition, the service offers rehabilitation to enable earlier discharge from hospital or to prevent hospital admission. You will work as part of a multi-disciplinary team, following initial assessment by a registered health care professional you will exercise initiative & judgement in care planning, goal setting, delivery, implementation & evaluation of care, working collaboratively & in partnership with others. You will contribute to enabling service users to reach their potential & develop knowledge to promote holistic care, including supporting patients who have longer term rehabilitation needs. Hours of work will include weekends and bank holidays.
