This role is a new and evolving post that will adapt and change according to the developing needs of the team and applicants should be prepared for that.
The Same Day Service Care Coordinator will support the Same Day Service Team in coordinating care to patients requiring same day services, they will coordinate access to services ensuring that patients are seen by the most appropriate clinician for their need.
They will signpost, provide advice and information, and ensure that planning is timely, efficient, and patient-centered.
As part of their role, they will proactively identify and work with people, particularly the frail and elderly and those with long-term conditions, who have regular same day need to ensure appropriate support is made available to them and their carers, signposting them to services or practice team members that can help them understand and manage their condition, and ensure any changing needs are addressed.

The Same Day Service Care (SDS) Coordinator will undertake the following key responsibilities in delivering health services.
a. Working in conjunction with the SDS Team, scan the SDS triage list to ensure that patients are dealt with by the most appropriate clinician, moving patients where appropriate into clinics
b. Signpost patients to the most appropriate service, be that internal or external
c. Swiftly work on patient communication campaigns where heightened need occurs due to nationally reported issues i.e. pandemics, heightened cases of illnesses etc
d. Provide support to the SDS Team by dealing with all administration, including responding to eConsults as directed by the clinicians
e. Monitor patients who are high users of the Same Day Service in order to see if there are further support measures that can be put into place
f. Assist clinicians in getting through to acute departments if patients need admission
g. Monitor and manage the visit list
h. Ensure the SDS suite is fully stocked with consumables and all equipment is working
i. Liaise with patients having difficulty getting to the surgery to see if there are options available that would avoid the need for afternoon visit requests (community transport etc)
j. In time, and following training, take basic observations from those waiting to be seen by a clinician
k. Organise and improve the process for patients sending in photographs for clinician’s review.
l. Contribute to public health campaigns (e.g., COVID-19 or flu clinics) through advice or direct care
J. Assist clinicians to ensure we are meeting QOF targets
