Occupational Therapist / Care Coordinator - Band 5
South Warwickshire Community Mental Health Team

We have a vacancy for a motivated and creative Occupational Therapist to join our team. The team work with service users with service mental health conditions across the South Warwickshire area, and are based in Leamington Spa and Stratford Upon Avon. This is an exciting opportunity to bring creativity to a dynamic and forward thinking team. There is scope to do individual and group therapies for our large patient group.
We are going through a transformation change within our community services, meaning that we are able to broaden our offer of specialist interventions for those we serve.
The successful applicant will work as part of a large multi-disciplinary team including other Occupational Therapists to deliver focussed interventions in the community. Our Occupational Therapists work alongside Nurses, Social Workers, Psychologists, Psychiatrists and Support Workers as well as our admin team. An Occupational Therapy priority pathway exists within the team to identify individuals with Occupational needs.
You will carry a caseload, and your workload will be split between OT specific assessment time and some Care Co-ordination under the Care Programme Approach for individuals who present with moderate to severe occupational performance issues.
There will be an expectation to carry out some generic roles in the team such as duty work.
