Job summary Senior Community Mental Health Nurse - A&D 1 x Full Time Permanent Post - 37.5hrs - Monday to Friday, 9-5 (based at Telford / Shrewsbury - to be determined by service need) An exciting opportunity has arisen for an experienced Senior Community Mental Health Nurse to join the Assessment and Diagnosis team within the Dementia Assessment and Support Service (DASS). We are in an exciting period of change due to the Dementia Transformation taking place across Shropshire, Telford & Wrekin. This role involves working closely with our Nurse Practitioners who assess and diagnose Dementia, to support the referrals management process and to provide community-based intensive input to our service users and carers who have more complex needs, whilst in the assessment process, and this will be driven by clinical need.
