As a Learning Disabilities and Autism Keyworker, you will play a pivotal role in supporting people and and adults with complex needs. Your primary responsibility is ensuring the voice of individuals, and their families are captured and heard and you will troubleshoot to ensure necessary support is in place, ensuring that other professionals and services work cohesively around the person to prevent further escalation, family or placement breakdown and admission into an inpatient setting. You will facilitate emotional and behavioural support, promoting effective communication, and foster positive relationships. A key part of the role will be to navigate complex systems, challenge the status quo where required including any potential admissions to hospital to ensure that all other options have been exhausted. The role demands a keen understanding of health and social care and education systems, coupled with the ability to coordinate multi-agency efforts for the benefit of individual.
