Main duties of the job
The Lead Nurse will provide clinical and professional leadership to the nursing team. The post holder will ensure nursing services are delivered effectively to the patient population.
Our ideal candidate will have drive, dedication, enthusiasm, and a positive attitude. They will provide effective and high standard care to patients including, all chronic disease management, wound care, cervical cytology, and other duties.
The post holder will line manage the nursing team in conjunction with the practice manager, ensuring high quality patient centred care is delivered and facilitate their professional development.
They will be responsible for leading and directing the development of new services and patient care pathways, ensuring they match the needs of patients.

Job responsibilities
Leadership and Management
* Provide overall line management for the Practice Nursing Team, ensuring effective team performance and staff wellbeing.
* Lead the planning and implementation of local guidelines, protocols, standards, and quality improvement initiatives.
* Act as a clinical leader in delivering nursing services, prioritising patient needs.
* Participate in and contribute to the practices clinical governance and management meetings.
* Develop, implement, and monitor annual appraisals and personal development plans for the nursing team.
* Manage staffing levels, nurse appointment systems, and ensure adequate nursing team cover.
* Oversee infection control, health, and safety standards, ensuring compliance across the practice.
* Manage and optimise the use of resources, including stock control and equipment.
Clinical Responsibilities
* Provide routine and complex nursing care in line with NICE guidelines and practice policies.
* Diagnose, monitor, and manage chronic diseases, developing individualised care plans collaboratively with patients.
* Administer immunisations, vaccinations, and travel health services.
* Perform wound care, therapeutic monitoring, health screening, and risk assessments.
* Deliver first-contact care, independently managing or referring patients as appropriate.
* Ensure accurate documentation of care and adherence to information governance policies.
* Provide professional guidance and support for safeguarding vulnerable patients and dealing with substance misuse or mental health concerns.
Staff Development
* Identify training needs and promote lifelong learning for the nursing team.
* Facilitate clinical education, one-to-ones, appraisals, and practice-wide learning events.
* Mentor staff and support professional development within the team.
* Promote integrated care by collaborating with the wider nursing team (e.g., district nurses and health visitors).
Quality and Service Improvement
* Develop and monitor quality markers and audit processes to promote clinical effectiveness.
* Participate in practice research activities, contributing to evidence-based practice and innovations.
* Co-ordinate and lead regular nursing team meetings to ensure effective communication and service alignment.
* Support the achievement of QOF and Enhanced Services to deliver high standards of patient care.
* Ensure the implementation of health promotion and chronic disease management services.
Patient Care and Advocacy
* Act as a key point of contact for patients, delivering patient-centred care and health promotion initiatives.
* Provide guidance on prescribed medications and promote compliance.
* Administer vaccines and other therapies, monitoring for effectiveness and side effects.
* Chaperone patients where required and ensure patient dignity, rights, and confidentiality are upheld.
Health and Safety
* Lead infection control practices, ensuring adherence to policies and providing training as needed.
* Manage the safe disposal of sharps, waste, and specimen handling.
* Monitor and maintain cleanliness and sterile environments within the practice.
* Identify risks within work activities and implement measures to mitigate them.
Communication and Collaboration
* Foster effective communication within the nursing team, practice, and wider healthcare network.
* Work collaboratively with external services to ensure comprehensive patient support.
* Contribute to the development and delivery of educational programmes for medical students, GP registrars, and the team.
Person Specification
Experience
* Significant experience in nursing.
* Strong background in chronic disease management and health promotion.
* Knowledge of practice management systems and NHS frameworks.
* Experience in primary care settings.
* Commitment to fostering a culture of learning and service excellence.
* Proven ability to lead and develop nursing staff within a clinical setting.
* Experience in conducting audits, quality improvement initiatives, and achieving clinical governance targets.
* Familiarity with primary care systems such as SystmOne and Ardens.
* Interpretation of spirometry results to support respiratory care.
Qualifications
* Registered Nurse with valid NMC registration.
* Evidence of ongoing professional development in line with NMC revalidation requirements.
* Certification in cytology and immunisation (childhood and adult).
* Additional qualifications in chronic disease management or leadership.
* Certification in travel health.
* Spirometry Qualification.
