Job summary We are looking for a qualified Occupational Therapist to join our Intensive Rehabilitation Service supporting Pathway 2 discharges in the South of Wiltshire based in Salisbury area. This service is based within inpatient bed based rehabilitation setting to provide patients an opportunity for focused rehabilitation after a hospital admission to enable patients to work towards their goals. You will work with a multidisciplinary approach, alongside physiotherapists, assistant practitioners, rehabilitation support workers, care staff and social workers to ensure the patients are supported to work towards their goals, in aim of returning home or into another setting. As a band 5 Occupational Therapist, you will be supported by a Therapy Clinical Lead and senior therapists within the team along with community therapists within the organisation to develop in your role to increase your knowledge and experience working with a diverse case load. To work within the integrated multi-professional team. To be accountable for assessing, interpreting, planning and implementing occupational therapy treatment and care to patients within professional guidelines, with the support of specialist team members and the Team Leader. To undertake assessments, care planning and evaluation. Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.
