This is an exciting opportunity to work within our dynamic and innovative LARCH service.
The LARCH Team is an in-reach care sector support service that responds to the changing needs of the care sector population. The goal is to improve health outcomes for people to maintain optimal health within the care sector, reducing unnecessary emergency admissions, and maintaining good quality of care in collaboration with community services and neighborhood working.
The team will coordinate their work across the county to reduce the number of avoidable admissions to hospital from care homes and to improve the experience of care home residents when they become unwell by working collaboratively with care homes to ensure residents receive the right care, at the right time, in the right place, by the right people.
Main duties of the job
The Practitioner will support the development, implementation, and evaluation of programmes and pathways of care and work autonomously to manage their workload, using a multi-disciplinary and multi-agency approach to embed high standards of care within the care homes in Somerset.
* Mentoring role for care home managers and LARCH colleagues.
* Health assessment reviews in home closures.
* Clinical skills training / competency assessment.
* Provide support and education to the PCN representatives and care providers supporting the Health and Wellbeing elements of the Enhanced Health in Care Homes framework.
* Treatment escalation Plan (TEP) education, quality auditing, and complex TEP support.
* Form part of the multidisciplinary team that responds to data and trends identified in the Quality Assurance Framework meetings for all care sectors, and other intelligence sharing platforms, to proactively address concerns and minimize potential escalation and impact to the wider system.
* Lead and support system innovation in relation to the prevention of hospital admissions and improving outcomes for those living in care homes, such as new models of care, championing roles of urgent crisis response, and Hospital@Home services. Promote and link with priorities around discharge, frailty, and system flow.

Job responsibilities
The service operates Mon - Friday 08:00 - 18:00.
The service works remotely from home, virtually and within PCNs, care provisions, neighborhood and community bases. You will need to be a car driver, with your own vehicle and business insurance. We offer a mileage allowance to cover the use of your car for NHS business, which includes mileage and depreciation.
This position is day shift, Mon - Fri 9-5 with currently no night shifts or weekends. We can offer flexible working solutions to ensure a work-life balance, and part-time hours may also be considered.

* Registered professional
* Evidence of relevant post-graduate specialist training or willing to undertake more complex supervision and training
* Evidence of continuing professional development

* Quality improvement experience with leading or participating in quality improvement initiatives
* Management qualification or experience
* Mentoring qualification or equivalent
Experience Essential
* Experience of delivering services within a performance management framework and of developing and implementing action plans in order to meet specific national and local targets.
* Experience in the use of IT platforms such as Word, Excel, Outlook, PowerPoint, and Teams.
* Ability to work as an autonomous practitioner.
Desirable
* Experience of working across organisational boundaries in a health care setting
* Experience of mentoring
* Awareness of Enhanced Health in Care Homes and Primary Care Networks
Additional Criteria Essential
* Compassionate - with the ability to communicate sensitively and empathetically.
* Ability to record and retrieve information on charts/paper and electronic patient records.
* A car driver with a full valid UK driving licence and business insurance in order to travel throughout the Trust to meet the needs of the service.
* Flexible and adaptable to meet the needs of the service.
* Good interpersonal skills are required to write person-centered Somerset Treatment Escalation Plans.

* Analytical skills to deal with interpreting complex clinical information.
