This is an exciting and unique opportunity to join the newly developed Forensic Intellectual and Neurodevelopmental Disabilities (FIND) Community Team in South-West London & St George's Mental Health Trust (SWLSTG).
The FIND Team will work closely with the newly developed Oak Unit, a Learning Disability and Autism (LDA) Ward, one of the four inpatient wards, which comprise the South-West London & St George's Forensic Service. Providing a safe transition for inpatients discharged from the Forensic Service is the Forensic Outreach Service (FOS) and the FIND Team will sit within FOS in order to provide specialist assessment, treatment and consultation to the Forensic Service in order to ensure a smooth, safe and clinically justified pathway to individuals with mental illness, offending history and learning disability with or without neurodevelopmental disorder.
The three forensic services of SWLSTG, South London and Maudsley (SLaM) and Oxleas together comprise the South London Mental Health and Community Partnership (SLP). The SLP's overarching goals centre on bringing care closer to home, improving patient pathways and outcomes by delivering excellent care in better environments. The SLP has a track record of delivering highly effective, clinically informed service developments in order to better meet the needs of service-users.
Main duties of the job
This role is part of a new and exciting service providing forensic community service to a specialist group who require expert interventions. The FIND service works with adults with a diagnosis of a learning disability (LD) and/or Autism (ASD) who also experience additional mental illness, co-morbid neurodevelopmental conditions, or behaviours which challenge and who have had contact with forensic services.
The successful candidate will work within the Forensic Directorate in providing support to individuals with learning disabilities leaving secure mental health services across the south London partnership - the core job role is the management of a defined caseload of patients. As a care coordinator, you will work collaboratively with other agencies, including police, MOJ, probation, housing, and residential providers (to name but a few), to ensure that service delivery is consistently of the highest standard, providing for safe and effective care and team support.
The successful candidate will be a qualified RMN or Learning Disability Nurse with sound experience in Mental Health, with knowledge and experience of forensic mental health service provision, offender care and be committed to team working.

To be responsible for the flexible management of a defined caseload designed to meet the individual needs of the client and the carers.
To maintain a high standard of individualised client care, recognising factors and events that are likely to affect the mental, emotional and physical well-being of clients/carers and provide advice and support when required.
To act as a care coordinator in assessing clients' needs and strengths through a collaborative approach, ensuring that all clients have care plans and that these are implemented and regularly reviewed/updated.
To develop and maintain a high standard of practice, based on assessment, through the use of a range of psychosocial approaches, within a health and psycho-social framework.
To communicate/liaise effectively within relevant agencies to ensure that an integrated programme of care is provided.
To be able to manage medication in accordance with the Trust Policy and the NMC Standards for administration of drugs.
To organize and participate in the admission, discharge and transfer of clients in accordance with the Trust Policies.
To provide clients and carers with verbal and written up-to-date information on the services offered by the Trust, how to express their views, complaints procedure and what they can expect from the CMHT.
To attend weekly clinical meetings to provide the team with relevant information in relation to caseload and to feedback on new assessments.
To supervise and monitor the therapeutic effects and side-effects of prescribed medication providing the client with clear advice and information.
To develop and maintain effective working relationships with in-patient facilities regarding admission and discharge of clients.
To facilitate carers assessments, provide the appropriate care plan to meet identified needs and to monitor efficiency.
