Discharge Co-Ordinator
Band 4 - 37.5 hours
The post will support wards to safely plan and discharge patients with the support required.
Discussions with patients and family members and the ability to use and refer to specialists teams in the community as required
Attend daily board rounds- to be aware of all the patients on your allocated ward area.
To liaise with the Band 6 supporting your team
We will support with any development staff member needs
We provide in house training relevant to our area .
We can access training from community teams
Regular supervision

The role of the discharge Co-ordinator is to provide support to wards, attend board rounds as discharge starts on admission. Ensure that all patients are discharged safely with the care they require. To be able to escalate any delays in care or discharge planning
To liaise with other team members for patients from care homes and patients that are approaching end of life
