Working creatively to help make sure that people in acute mental health crisis have the right care in the right place at the right time is critical to success in leading the implementation of our Transfer of Care Hubs (ToCHs). Central to this is working closely with inpatient and crisis/intensive services across AWP to embed the principles of GIRFT (Get it Right First Time).
The aim is to support people to be admitted to hospital at the right time and to help people to return home at the earliest opportunity.
The intended outcome is that we are able to care for people in inpatient settings as close to home as possible and thereby achieve our ambition to reduce inappropriate out of area placements.
Main duties of the job
We will have two ToCHs - one covering Bath and North East Somerset, Swindon and Wiltshire and the other covering Bristol, North Somerset and South Gloucestershire.
The ToCHs will work closely together to manage across our Trustwide bed base and to provide each other with peer supervision and support. You will be an experienced manager, with clinical experience in the acute mental health pathway (inpatients or community).
Your role will be to establish and manage a ToCH, working closely with your counterpart to align processes and cross-cover. This is a unique opportunity to set the tone and culture right from the start.
Our vision is that the ToCHs become multi-agency in nature. You will work with the local Health and Social Care system to attract Local Authority, Voluntary and Community Sector and ICB support into the ToCH. Reporting directly into the Divisional Director of Operations, this is a critical and high profile role within each Division.

Job responsibilities
* To provide operational management and leadership for a Transfer of Care Hub.
* To work within the Division to implement best practice in mental health acute flow management.
* Responsible to the Divisional Director of Operations for the effective management and operational delivery of flow across multiple sites and services, working within a context of continuous improvement and ensuring the delivery of integrated care pathways and partnership working across health and social care. To set up and manage the Transfer of Care Hub ensuring that effective structures, processes and accountabilities for the management of services are in place.
* Support the Out of Area Placement manager in oversight of contract beds and quality assurance.
* Lead the team in committing to achieving a consistent reduction in Out of Area Placements.
* To liaise with service users, carers, staff and partners across health, social care and the voluntary and community sector, ensuring that there is shared understanding of service provision and commitments.
* To be responsible for the delivery of Health and Safety responsibilities as required by statute, regulation, legal obligation and AWP policies within the defined geographical area.
* To ensure staff compliance with processes to protect Service Users, Carers, Communities and Vulnerable Adults.
* To ensure compliance with legal and statutory duties related to Mental Health Act, Mental Capacity Act, Police and Criminal Evidence Act.
* To ensure that services are planned and delivered with sensitivity to the diversity of the communities it serves.
* To work closely with colleagues to ensure due diligence processes are followed to give assurance as to the quality of out of trust placements.
* To ensure that individuals admitted out of trust or to Out of Area Placements are receiving appropriate care and are transferred as close to home as possible as quickly as possible.
* To oversee management and monitoring of our contracts with out of area private providers and to make recommendations to the Division and Executive for flex of the contract beds as required to ensure people are cared for as close to home as possible.
* To undertake investigations of serious untoward incidents, complaints, disciplinaries and grievances.
* To ensure that meaningful staff involvement takes place within the area managed so that commitment and ownership are maximized and staff have the opportunity, where possible, to influence service provision.
* To ensure clear Business Continuity Plans are in place.
* Because of the seniority of this post the postholder may be required to participate in on-call arrangements, after suitable training.
Person Specification
Essential
* Educated to degree level or ability to demonstrate equivalent level of knowledge and experience.
* Experience of successfully managing significant and complex change.
* Detailed working knowledge of the NHS, local government policies and mental health legislation.
Desirable
* Commitment to achieve positive service outcomes for service users and improving their experience of healthcare.
