Medication Review and Optimisation Respond to pharmacy queries, including clarifying doses and/or products and providing appropriate alternatives when availability issues arise. Discuss medication issues with patients as needed, including adverse effects, interactions, and queries related to over-the-counter remedies. Conduct medication reviews and review repeat medications for new patients registering with the practice, suggesting GP medication reviews where appropriate. Face-to-face and video consultations with patients will include discussing medication issues as needed and conducting medication reviews. Medicines Reconciliation Review secondary care requests for new medications (as communicated by discharge summaries, outpatient letters, etc.), raising queries with relevant GPs and addressing any inappropriate prescribing requests. Handle anticoagulant start/stop requests from secondary care and contact patients regarding missed INR appointments. Unplanned Admission Prevention Collaborate with the practice team to identify patients at risk of unplanned admissions. Medicines Information / Education Monitor and inform colleagues about ongoing prescribing issues, including new guidelines, products requested by secondary care, and any manufacturing or supply problems. Advise on cost-effective prescribing and budget issues. Prescribing Systems and Policies Assist in identifying patients in need of medication review and developing systems to meet these needs. Work with GPs and practice teams to implement safe and efficient prescribing policies. Medicines Safety Identify national and local policies and guidance that impact patient safety regarding medicines, including alerts, product withdrawals, and emerging clinical evidence. Liaison with Community and Hospital Pharmacies Support the integration of CAM Medical Primary Care Network with community and hospital pharmacies, managing patients at risk of medication-related issues upon discharge. Administrative Duties Across Three Practices Coordinate pharmacy services across three practices, ensuring effective communication among teams. Maintain accurate records of medication reviews, consultations, and patient interactions, ensuring compliance with practice policies and procedures. Attending training sessions and meetings for pharmacy and PCN staff across all practices. Support the implementation of administrative processes that enhance the efficiency of pharmacy services.
