About the role - HOPE Lead Practitioner Role (Health, Outreach, Prevention and Engagement) This an exciting opportunity to develop a team within a newly commissioned drug and alcohol service in the City of Sheffield The HOPE Team delivers training internally and to partner agencies. Facilitates the needle Syringe Provision across Sheffield and works with the City's most vulnerable and marginalised clients with enhanced needs. Working hours will be - 9am-5pm Monday to Fridays however with this role being part time the successful candidate can choose the three and a half days. This is an office-based role with the possibility of being able to attend events/ drop in clinics. Driving licence not required however preferred. Next steps will be a face to face interview. Key duties: Manage a caseload of service users, completing and reviewing high quality and comprehensive assessments, risk assessments and support plans, including liaising with family members, significant others and professionals / agencies as appropriate. Support the Service/Area Manager to develop, deliver, monitor and improve processes, control systems and work environments to meet quality requirements and contractual needs including KPIs.
