Job summary We currently have a permanent vacancy for a full time experienced band 6 nurse to work in the Older Peoples Mental Health Intensive Home Treatment Team in Oldham. Older Peoples Services are changing to meet the needs of people experiencing serious mental health problems in the community who are over 65 years of age and those people under 65 who or diagnosed with early onset dementia,, their partners, carers and families. We are looking for an enthusiastic, driven nurse who can work autonomously in the community, complete full mental health assessments, manage risk in the community, while also being an integral part of our multidisciplinary team. We are a small dedicated team of nurses, and support workers who work closely with our Consultant Psychiatrist, our colleagues in the community mental health team, care home liaison team, memory assessment team and our day hospital, Orchard House. Main duties of the job This role will involve nursing people with moderate to severe mental health problems at home as an alternative to admission to an acute hospital bed. In addition, this post will involve Gatekeeping admissions to inpatient services, as well as facilitating early discharge / leave pathways to support discharge from inpatient services. The successful applicant will have sound knowledge and experience in risk assessment, formulation and management. Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness. A knowledge and understanding of Mental Health Act '83 and the Mental Capacity Act are essential to this role.
