The Hospital Discharge Team are based at the Diana Princess of Wales Hospital. The Hospital Discharge Team is responsible for the safe and effective discharge of patients from hospital into the community and also the prevention of admission to hospital. The Hospital Discharge Team works in partnership with the Acute Trust ensuring effective prevention and discharge pathways are followed. The postholder will provide professional support to those staff working in the Hospital Discharge Team. The team operates 7 days per week working between the hours of 8.00am and 8.00pm.

Hours: 37.5 hours per week
Rota pattern: You will be required to work a rota pattern which includes weekends and Bank Holidays.

Salary Range: £35,527 - £42,663 per annum.
You will work as part of the Hospital Discharge Team, ensuring safe and effective hospital discharge is effected by way of assessment, planning, co-ordination, and communication at the interface between hospital and community in order to enable a smooth transfer of care between Secondary and Community Care. Assessment of Complex Health and Social Care needs is an integral part of the role. You will proactively manage episodes of care to reduce the length of stay in hospital, promoting effective channels of communication between all agencies involved in the continuity of patient care.
You will also be facilitating discharges for EOL patients in area and out of area. You will be able to support individual decision making in respect of end of life care and preferred place of care and death. You will also provide advice and support to individuals and their carers throughout this process.
