Main duties of the job About the Development Role: As a Continuing Healthcare Practitioner within our Liaison Care Delivery Team, you will be supported to develop the knowledge and skills to become proficient inundertaking and reviewing all elements of the Continuing Healthcare process in line with the NHS CHC Framework, including completing assessments using tools such as Decision Support Tool, Fast Track and Adult Needs Assessments.
