To work within the cardiac rehab team to deliver a safe and effective cardiac rehab programme for a people with a variety of cardiac conditions. DETAILED DUTIES & RESPONSIBILITIES Communication To effectively communicate complex information in a simple way to ensure patients and carers understand their condition and how to maximise their rehabilitation outcome. This requires experience in using motivational techniques and positive, non-judgemental approach. To establish and maintain rapport with patients and carers. To overcome barriers and source solutions towards effective communication, eg. to patients with hearing or visual impairments, anxious individuals, etc. To facilitate and be part of a well-coordinated MDT for patient centred care. To maintain accurate, comprehensive, and up to date clinical documentation, in line with legal and service requirements. To communicate and/or escalate concerns to the appropriate clinician, including senior members of staff, consultants and GPs, via the most suitable means, email, telephone, letter. To establish and maintain healthy working relationships within but not limited to the team. To develop motivational strategies in order to support and inspire patients, carers and other staff members. To deliver presentations and in-service training and contribute to professional development forums. To promote the Cardiac Rehabilitation service and to ensure a consistent approach to patient care. Analytical Tasks To undertake comprehensive assessment of patients with a range of clinical conditions, including those with a complex presentation, using analytical skills. To utilising a range of intervention skills and options to formulate a comprehensive, evidence-based individual management plan. To be able to analyse and interpret clinical and social information in order to make appropriate recommendations for other professional intervention if required. To integrate this information into patient management. To evaluate the patients understanding in order to gain valid informed consent and have the ability to work within a legal framework. This includes patients who lack capacity to consent to treatment. Planning and Organisational Skills To organise and manage a large caseload, with clinical priorities and delegate responsibilities to other staff as appropriate. To assist in the mentoring and training of support of staff and students. To co-ordinate and produce reports as requested for the area of work. To demonstrate a sound understanding of clinical governance and risk management and apply them at work. To deputise for other senior staff in their absence Physical Skills To be able to lead a group or a one-to-one exercise session. This includes cardiovascular, strengthening, flexibility and balance exercising. To be able to move and handle patients and pieces of equipment in a safe and effective manner. To maintain up to date training and knowledge of moving and handling, basic life support skills and immediate life support skills. To adhere to service policy including competence to use equipment. Responsibility for Patient Care To be professional and legally responsible and accountable for all aspects of your own work, including the management of patients in your care. To ensure a high standard of clinical care and to assist staff to do similarly. To be responsible for identifying patient goals and development/monitoring of treatment plans. To work within professional and organisational guidelines. To strive to maintain standards of practice and undertake regular training as required. To be responsible for the safe use of equipment by patients under your care. To provide relevant advice, to other members regarding the management of patients. To participate in referral, assessment, intervention, and evaluation in order to meet patients needs. Policy and Service Development Implementation To assist in identifying, maintaining and monitoring quality standards for clinical practice within the service. To use appropriate networks to share information, research and good practice examples. To demonstrate clear understanding of the role of national agencies and legislation and systems for improving the quality of care and work within these frameworks to deliver a high quality physiotherapy service. To ensure that work is conducted in accordance with organisational / service policies and guidelines and within Health and Safety Regulations. To contribute to training and development programmes in the current service area Responsibilities for Financial and Physical Resources To be aware of budgetary constraints. To assist the line manager in the delivery of a cost-effective service and be aware of performance measurement criteria. To be involved in the collection of appropriate data and statistics for departmental audit purposes. Responsibilities for Human Resources To train and supervise/mentor other members of staff as well as students. To contribute to the training and development of others as and when required to improve service provision. To take responsibility for own development and actively participate in supervision. To maintain own continuous professional development (CPD). To be an active team member and participate in in-service training programme by attendance and involvement at staff meetings, training sessions, external courses and reflective practice. Responsibilities for Information Resources To be responsible for collating data as per clinical area. To be responsible for the recording of clinical information in relation to patients and clients and their treatment programmes ensuring confidentiality is maintained at all times. Responsibilities for Research and Development To actively search out and incorporate evidence in to practice. To contribute to the measurement and evaluation of the service. This may involve the use of audit and outcome measures, in association with other services.
