An exciting opportunity has arisen within NHS West Yorkshire ICB Wakefield place (ICB) to work within the continuing healthcare team undertaking comprehensive assessments, reviews and determining the level of care required within the National Framework for NHS Continuing Healthcare.
We are looking to appoint a registered nurse to join the continuing health care team.
The post holder should be a Registered Nurse (Learning Disabilities) and have knowledge and experience of working with the National Framework for Continuing Healthcare, (revised 2022).
The post holder will be responsible for completing assessments and reviews of individuals with learning disabilities who are eligible for Continuing Healthcare funding and to provide case management to a small cohort of people whose care is funded entirely by the CHC team.
An ability to work collaboratively with a wide range of teams within the ICB and other partner organizations will be required to ensure the appropriateness of assessments and care plans e.g. provider services, acute hospital, community learning disability teams, Social Services, CMHT, education services.
The post holder will liaise with other agencies and refer as necessary if deemed appropriate, maintaining good relationships with colleagues.
A comprehensive training and mentorship program will be offered to the successful candidate.
Main duties of the job
The post holder will be responsible for:
1. Undertaking NHS Continuing Healthcare/Funded Nursing Care assessments, care planning and the evaluation of packages for patients eligible for NHS Continuing Healthcare.
2. Support the delivery of personalised commissioning to include transforming care work (Care Programme Approach, Care and Treatment Reviews, s117 aftercare arrangements), Personal Health Budgets, the coordination of complex cases (incorporating Deprivation of Liberty Safeguards (DOLS) and clinical input into Court of Protection where appropriate).
3. Working independently within codes of practice, standard operating procedures and professional guidelines, providing strong clinical leadership ensuring the highest standard of clinical assessment.
4. Ensuring a patient focused, collaborative approach to identifying health needs and facilitating the most appropriate and cost-effective care pathway for individuals.

Job responsibilities
1. Support complex case management and work in collaboration with other health professionals and social care staff.
2. Deputise for the band 7 nurses.
3. Complete MCA assessments and Best Interest decisions where appropriate. Work within the DoLS framework, identifying and responding to the need for DoLS authorisation.
4. Offering continuing support and advice to family and carers in complex and sensitive situations.
5. Developing and maintaining effective and appropriate communication networks with health and social care professionals.
Person Specification
Experience
* Minimum 2 years post qualification experience as a Registered Nurse.
* Significant experience of both community and hospital working and possess a sound understanding of the NHS CHC national guidance.
* Experience of working in a hospital and community setting.
* Extensive knowledge of the Mental Capacity Act and Deprivation of Liberty Safeguards.
Skills & behaviours
* Computer literate with an ability to use the required systems/Microsoft IT office packages.
* Has a planned and organised approach with an ability to prioritise their own workload to meet deadlines.
* Able to exercise independent judgement where appropriate and problem solve.
* Must be a car driver with access to own vehicle.
Qualifications
* RNLD.
* Educated to degree level or equivalent in experience.
* Current and relevant NMC registration.
