You will be working within an Adult Disabilities integrated health and social care team and supporting community partners to improve the health of people with disabilities. About the team As a Community Nurse you will work alongside paid and family carers to support people with learning disabilities to access mainstream health services by supporting clinicians to make reasonable adjustments. Your skills in adapting and creating the right approach for the individual will help bridge the gaps and reduce the risk of avoidable health conditions. You will be supporting young adults with a learning disability aged 18 to 25 years, working with young people and their paid/family carers to understand the challenges and rewards that moving through adolescence into adulthood can bring. You will work alongside colleagues such as school nurses, GP’s and mainstream health partners to enable access to healthcare with reasonable adjustments. About you We require people who can be creative and flexible to different approaches for different situations as no day in community learning disability nursing is the same. As the post is in the community, you must hold a valid driving licence and will need to be able to demonstrate the ability to travel to community visits and across to the 2 main HCC sites in Hemel Hempstead and Stevenage in a time efficient way. As a Band 6 Nurse you will be working alongside nursing colleagues to triage and managing referrals to the service. You will require a need to demonstrate positive behaviour support and how to reduce health inequalities to the people we support. This is a great development opportunity if you are looking to step up from a band 5 position into a band 6. This is also an opportunity if you fancy developing your skills somewhere new. You will need to hold a Nursing qualification in Learning Disabilities and have an active nursing registration and be able to demonstrate a working knowledge of Safeguarding.
