About the Role: As a Care Assessor in the Complex Care Team, you will be at the forefront of our efforts to support adults with the most complex needs. You will support the diverse caseload from across the team working with a variety of residents with high-risk and high-needs working closely alongside the Advanced Practitioners. You will carry out high quality holistic assessments, using strengths based, person-centred, community focused, and multi-agency interventions to bring about positive change through developing effective relationships and to secure the best outcomes for people and carers. Your expertise will be crucial in supporting adults through challenging situations, working with people with multiple long-term conditions and co-morbidities around drugs, alcohol, mental health, and homelessness; complex safeguarding, and court work. Key Responsibilities: Support the management of complex cases with a focus on holistic, strengths-based, and person-centred approaches Gather information to support legal cases, court reports, and safeguarding investigations Develop and maintain positive relationships with clients, families, partners and stakeholders What We Offer: A supportive and collaborative team environment.
